Sound like fueling issue, either bad/water contaminated fuel or an air leak after the throttle body so check gaskets on manifold and throttle body etc.

Also stop engine, leave a minute or two and check brake servo is not leaking its vacuum as well. If it is then the engine will be sucking in un metered air giving it a lean mixture causing bad idle and the stalling issues you have experienced as well.

does this happen when cold? mine does this all the time when its warm, im pretty sure its the idle control valve sticking shut not letting enough air into the inlet on start up to achieve the right fuel/air ratio. take it off and clean it, if thats no better get a new one, theyre about 80 quid

If it was the idle control valve then the engine would rev but as it dies when he opens the throttle suspedt it is not at fault.
