Lowepro are the daddy.
Crumpler make some funky shit too.
Have a look at a Lowerpro Orion Trekker II, you should be able to get all the stuff you listed in there

I've just ordered a Lowepro Slingshot 200AW from the Digital Camera Company, they seem to do some good deals ....... Also a new 4GB memory card . Preparing for two days in the welsh forests for Rally GB in December

i have two the first being a lowepro Nova 5, its quite big but i can fit 2 bodies, 4/5 lens and flash gun, the interior sections are removable so you can jig them about to suit your stuff. i mainly use that for a big shoot like if i am doing a fashion shoot on location and need to shoot film, medium format and digital.
otherwise for day to day use i have a kamala bag which is big enough for my 10d with battery grip and 135mm lens fitted and space for a 70-300mm lens aswell as pockets for spare batteries and cf cards. ideal really when you dont want to lug loads of stuff but may want a telephoto with you.
