Erratic/high idle and problem starting

I have a Mk2 VTR which has recently been having problems starting (especially when cold). When I start it, the revs rise and then instantly drop unless I hold them up. If I hold up the revs for 30 seconds or so and then slowly release the throttle it will often idle ok. After that, when coming to a stop the revs will either drop very low (and sometimes die) or will erratically shoot between 1000 and 3000.

I'm thinking it's either the ICV or the coolant temperature sensor, but was wondering if anyone could identify with this problem and what could be the cause?
