Default clutch on way out???

i beleive its the clutch release bearing makes a very loud not healthy sounding grinding noise unnder acceleration in 1st ad 2nd gears not so bad in the rest... also if u sat inn neutral with engine running put clutch in and bring it out slowly you can hear a bit of a rattle just wondeering if any1 can pinpoint my problem? cheerz
