Its a router problem mate, guy at work had a simular problem.
Seriously you either have to be pretty nerdy to do it off your own back or just call xbox support ask them to resolve it, after all thats what you pay your live memebership for.
It wont take long and will resolve all the problems. I will host a game, you call them, i will keep sending invites and dont hang up the phone until you have connected twice to the game loby.

They didnt, hes a networking guy so he pissed about with the settings and fixed it.
I think its the crossing of ISP's and router settings not giving correct access.
whats your ISP and i will have a fish about.

1) Turn off any portforwarding
2) Turn off any DMZ's
3) Choose the most recent certified version of firmware from the list here that matches the version number of your router http://kbserver.netgear.com/kb_web_files/n100981.asp
Load it into the router and try again

Fishes something out....
I take it you know how to get into the Admin panel on your router ?

My Live connection was created by a pure technophobe....a.k.a me..
Got a massive ethernet cable going out of window....down the outside of the house and into the living room and connected to back of pc...so whenever I turn on xbox....the pc switches itself on.....pow

mmm looks like what i quoted above is the same advice for that router.....
If you struggle, why dont you install VNC, give me your IP, i will connect, change the settings and then we can test it....
If it works you uninstall VNC and reboot you router so you get a new IP incase you dont trust me to have it (which they is fuck all i can do without VNC installed even if i wanted to) Would take 5mins max, to download, install, me remote in and hopefully resolve.
who is your internet provider?
